In addition to the fine the company was ordered to pay costs of £218,000.

The court heard that Mr Cameron was killed after being crushed between a large amount of debris and the powered roof support (PRS) system. The PRS system was subject to a faulty valve which had become defective and worn.

Since 2008 there have been two other deaths at the Killngley colliery centre – one due to a rock fall and the second to a roof collapse.

UK Coal were quoted as saying they deeply regretted any injuries that had been sustained and that there thoughts were with Mr Cameron’s family at this tough time. They have also promised to overhaul safety procedures with a new management team.

“The historic level of fatal accidents is totally unacceptable to the new management team and a full review of health and safety has been carried out across the business,” they said.

“Significant progress has been made and safety remains our number one priority.”
