Premature twins die after “failings” in care, according to a coroner.

A coroner has ruled that an overdose of morphine played a part in the death of two babies in Stafford Hospital in November 2010 – aged just one day old.

The twins were born 13 weeks early on 30 October 2010 and the cause of death was given as “complications in extreme prematurity” but the coroner did acknowledge that the babies had received “sub-optimal treatment”.

Although the hospital had followed procedures after the premature birth of the twins there was confusion over how much morphine should be administered after the birth. 

The deaths of the twins was “totally avoidable” according to their mother Ami Dean who believes her children would still be alive had they been at another hospital.

“[Their deaths] were down to human error, which is something I cannot cope with,” she said.

“I could have coped with them dying from prematurity, as that would have been nobody’s fault.”

Richard Follis, the family’s solicitor believes a “series of errors” led to the twins’ deaths and the family are pursuing a personal injury claim against Mid-Staffordshire NHS Foundation Trust.
